I had faced a similar problem after upgrading to Windows 10.
Solution:
Spoilers the folder Documents in the properties folder of The Witcher 3 in the Security, click Modify and choose Full access, Change, Write.

Checked out the Witcher 3 (steam version) on Windows 8.1 everything is going and saving works great.But with tens as it does not want to remain at all in some.Any help would be appreciated and I will have to return the incentive game ((( which of course did not want to do. About I uncheck read only after starting the game or restart the computer the check mark again stands in its place. I think to put Windows again but without the updates, if that does not help.
Similar problem. The Witcher 3 v. 1.31. Played 100 hours and then stopped to see a piece of paper right on the screen when saving., of course like saves. In folder Documents\Wicher3 does not work uncheck read only, from which the game cannot make changes to the subfolder with the saves. And then I saw bottom left the message that Management and folder access is blocked built-in GUARDом Windows.
