1. When Llope,
2. With places of power-a one-off.
3. With some game item.

Apparently you have some sort of crap with the game,this usually happens due to some kosyachny fashion.
Check the cache on steam.
You experience given for quests,not skill points, 1,2,3 points instead of 100 or 500, because you LVL very high and that's all.

For completing tasks and winning battles Geralt receives a different amount of experience. Portions of 100, 200, 500, etc. units. These amounts add up and accumulate, irregular leveling up Geralt. Clearly shows only those portions of the experience that we receive for the job. Each new level brings exactly one point of experience. First 1000 experience gives the transition to the next level, after level 10 to reach the next level you need to accumulate 2000 units of experience. Accumulated so (and also those in places of power) skill points the player assigns, in its sole discretion
