1. Download from here http://blog.metaclassofnil.com/?tag=dsfix DSfix, from Filehorst mirror. Upload files to the DATA folder.
2. Download DSMfix http://www.nexusmods.com/darksouls/mods/28/? latest version. (dsmfix09) and unpack the same way as DSfix. It is important to understand that this is an add-on to DSfix.
