Gamepad problem in VATS (Fallout 4)

I play on my computer with a gamepad. When using VATS, I can only change a limb once, for example, if the first time I was unable to switch from body to head, then this cannot be changed in any way, I have to re-enter VATS. Maybe someone knows how to solve this, otherwise opponents are running around like devils, so also VATS does not completely stop time, which only complicates the matter.
