Game difficulty (The Witcher 3: Wild Hunt)

I did not find a description for each complexity, add someone in the know.
There are four difficulties:
-Story
only -Story and fights (more enemies?...)
-Pain and suffering (more enemies?, can't be healed by meditation...)
-Death (Apparently permanent death, can't be healed by meditation.. .)

Who chooses what?

On the fourth I play, on Death!. It's not hardcore. I mean, after death, the game goes on, even though they write in the menu that something is not compatible. But they kill quickly. died a couple of times, jumping off the house =( But, again, the game can be restarted. It turns out, not permanent death.

to death - as I understand it, as in the second part, if you are killed, then you will start the game again, i.e. you have one life.

No. To Death! - a more difficult version of the third complexity. Like Dark difficulty in the second part. There is no permanent death.

I chose Pain and Suffering. The first time I felt the difficulty was when I was killed by drowners while trying to recapture the treasures. In two hours, I pumped a little, took two skills from the 4th branch - restoring health out of combat and increasing health by 500 units, killed wolves - they drop the liver of the beast and raw meat (quite often), which can be eaten. Haven't experienced any problems yet.
