The creation of icons. (The Witcher 3: Wild Hunt)

Question to professionals: how to create new icon? Create an image in png, with parameters identical to the icons of the game, Repack the archive, but the game does not see it, only an empty rectangle. If anyone can help I would be grateful. Sergey.

What kind of icon is it? A new icon of what? The executable of the game or something in the game?

I think the icon items in inventory, like the sword of a thread or the armor, some people have nothing else to do than to split hairs with icons that only see in the inventory.
