On disk for PS4 should have been an appropriate inscription about its presence. Inside the box was supposed to be a separate code to activate it (+ the code for Shao Kahn, once you have pre-ordered). Contact where you ordered the game.
This is important: the code for Scarlet were ONLY available for physical copies. Digital copies for the PS4 it is not and should not be the default.
