To play as your heir, you must die as your original character - unlimited health will prevent you from dying. I'm not sure if you can still die of old age, but it could take up to/more than 60 in-game years. To get this achievement, you must first get married and then have an heir.
You can have an heir at any age, but I would recommend not to go over 41 to make sure your heir turns 18 before you hit 60 and the occasional chance of dying of old age.
