Unlocking door break items. (Medal of Honor: Warfighter)

I know that if you type 4 skull icons i.e. A well-aimed headshot will open a new item for breaking doors. What does the red skull icon mean? I can't unlock the tomahawk in any way. Already shot enough and white and red skulls and counted all alone. Maybe someone understood the pattern?
