Geralt does not attack. (The Witcher 3: Wild Hunt)

Greetings to the fans of the witcher. The problem is this, the battle begins, a fast attack and a powerful one works, when you press TAB, the familiar menu of signs opens to everyone, but sometimes my character just goes into defense mode, does not react to an attack / powerful attack, and in particular does not I can go to the signs (TAB), this "glitch" passes the same way, suddenly. Please help.
